==== Initial outbreak ==== In December 2019, an outbreak of coronavirus disease 2019 (COVID-19) was first identified in Wuhan, Hubei, China; it spread worldwide and was recognized as a pandemic by the World Health Organization (WHO) on March 11, 2020. The first confirmed case in Canada was in Ontario—reported on January 27, 2020. == Role in gluconeogenesis == Fatty acids from lipids are commonly used as an energy source by vertebrates as fatty acids are degraded through beta oxidation into acetate molecules. This acetate, bound to the active thiol group of coenzyme A, enters the citric acid cycle (TCA cycle) where it is fully oxidized to carbon dioxide. This pathway thus allows cells to obtain energy from fat. To use acetate from fat for biosynthesis of carbohydrates, the glyoxylate cycle, whose initial reactions are identical to the TCA cycle, is used. === Environmental contaminants === Since American kestrels are carnivores, toxic chemical runoff ingested by their prey can concentrate at high levels in their blood. Wild kestrels are subject to immunomodulation, or an altered immune response, to polybrominated diphenyl ethers (PBDEs), a group of industrial flame retardants that may leach from factories into the environment. When PBDEs accumulate in body tissues of kestrels, the T-cell mediated immune response decreases in efficiency. As a result, kestrels that ingest PBDEs may not respond sufficiently to viruses or other invading microorganisms. In addition, certain PBDEs may suppress the growth and development of the spleen and bursa in American kestrels. While PBDEs can affect immune response and suppress growth of certain organs, they can also affect the thyroid system of American Kestrels. Exposure to PBDEs in vivo can alter the thyroid system and retinol concentrations in kestrels. This leads to oxidative stress, lipid peroxidation, and changes in glutathione metabolism. These systems are important in early development, growth, regulation of metabolism, thermoregulation, and reproduction. Oxidative stress is also known to contribute to cancers and neurological diseases. Exposure to polychlorinated biphenyls (PCBs) might also affect American Kestrel reproduction. It was found that PCBs affect the function of carotenoids in kestrels. This led to changes in coloration, especially during breeding season for adults. I’ve always been a fan of much of the earliest electronic music. I’d say obviously from that era Brian Eno, Tangerine Dream, Synergy, Isao Tomita and Vangelis all influenced me. When I was growing up in New Zealand, I remember seeing a BBC clip of Emerson Lake and Palmer performing “Fanfare for the Common Man”. I just thought that huge analog synth looming over Emerson looked and sounded so cool -- I was hooked. I still find something totally unique about the sonic textures many of the early synth pioneers were able to create. At a time when a polyphonic synth setup could cost as much as several houses, I think it influenced the amount of time and thought invested in crafting unique sounds. On the defunct Half-Life website, his function was described as follows: "Kelly did all of the music and sound effects for Half-Life, and wrote sound code to create character speech and DSP reverb effects." On Valve's official website circa 1998, his function was described as follows: "Kelly, formerly a product unit manager at Microsoft, has a programming background that includes consumer multimedia, database engines, and networking. He created all the music and sound effects for Half-Life." == Treatment == Surgery, if feasible, is the only curative therapy. If the tumor has metastasized (most commonly, to the liver) and is considered incurable, there are some promising treatment modalities, such as the radiopharmaceuticals Lutetium (177Lu) DOTA-octreotate) and 131I-mIBG (meta iodo benzyl guanidine) for arresting the growth of the tumors and prolonging survival in patients with liver metastases, though these are currently experimental. Chemotherapy is of little benefit and is generally not indicated. Octreotide or lanreotide (somatostatin analogues) may decrease the secretory activity of the carcinoid, and may also have an anti-proliferative effect. Interferon treatment is also effective, and usually combined with somatostatin analogues. As the metastatic potential of a coincidental carcinoid is probably low, the current recommendation is for follow up in 3 months with CT or MRI, labs for tumor markers such as serotonin, and a history and physical, with annual physicals thereafter.
